Jarme Home and Healthcare Services Corporation is a minority woman owned and licensed New York State DOH Home Healthcare Agency. Our office is located in the Bronx, but our services span across the five Boroughs of New York City and Westchester County. Our Services includes; Nursing Sevices, Home Health Aides Sevices, Personal Care Services, Social Work Services, Physical Therapist Services Occupational Therapist Services and Speech Therapist Services. We also provide Service Coordination ( SC ) , Community Transitional Services ( CTS ), Home and Community Support Services ( HCSS ), Independent Living Skills ( ILST ), Moving Assistance( MA ), Respite Services( RS ), and Wellness Counseling( WC ) under the Nursing Home Transition and Diversion ( NHTD ) waiver program. This is a Home and Community Based Services ( HCBS ) program administered by the New York State Department of Health ( DOH ) to assist individuals with disabilities and seniors toward successful inclusion in the community, especially participants living in the Nursing Home. Since no two clients are the same, Jarme provide a customized plan of care adjusted to your needs and Health condition. we work greatly with patients family and Physicians to ensure that their recovery at Home is achieved with dignity.
